Insurance Coverage for Ketamine Treatment
We are one of very few ketamine infusion clinics that accept major insurance plans for patients who have a valid pain diagnosis or are eligible for SPRAVATO® treatment. We are always working to expand access to ketamine therapy by growing the list of carriers who will work with us in covering this innovative, life-saving treatment.
Your in-network benefit coverage for care provided by Ketamine Wellness Centers is determined solely by the provisions of your specific medical benefit plan. Ketamine Wellness Centers may not participate in every product offering from the health plans listed and participation does not guarantee what coverage your plan will allow.
Disclaimer: Ketamine is FDA approved for anesthesiology, but its use in the treatment of mood disorders is still considered “off label,” severely limiting coverage of IV ketamine infusions.

Ketamine infusion therapy for depression, post traumatic stress disorder, obsessive compulsive disorder, and other mental health conditions is considered an “off-label” use and is not a service that is typically reimbursed. As such, patients are expected to cover the costs of their treatments.

Some carriers will cover a portion of the costs associated with ketamine infusion for particular diagnoses. However, reimbursement is entirely dependent upon individual medical plans and in/out-of-network stipulations. Our clinics will assist with verifying your health plan and outlining what your costs will be at the time of treatment. Our Billing Coordinator can assist all prospective and current patients in navigating their treatment coverage and reimbursement needs as it pertains to ketamine therapy for pain conditions.
We are happy to provide a superbill upon request so that you can submit a claim to your carrier for reimbursement. After rendering service, KWC’s internal insurance consultant can assist patients with seeking reimbursement for a small fee. Please call us at 855-KET-WELL for more information.

- Medicaid/Medicaid replacement or supplement policies
- Medicare/Medicare replacement or supplement policies
- Tricare/Tricare Prime/Veteran’s Administration (VA)/CHAMPVA.
